Special Programs to Help You Pay
A sliding fee scale based on income and family size is available at most health centers. We also have special programs that may allow certain patients to receive free family planning services.

We may need to verify your income to see if you qualify for discounts on your services. Please bring supporting materials about your income to your appointment. These can include a copy of the most recent tax return, a recent pay stub (no older than three months), or other documents including bank statements, court-ordered payments, unemployment benefits, worker’s compensation, pension distribution, social security benefits, W-2 or 1099-MISC forms, proof of income letter from employer.

Please come to your appointment even if you don't have access to these documents, as we can still see you and may be able to offer other programs to cover the cost.
