Insurance & Payment Options

We're committed to providing affordable, high-quality care.

We accept many different types of insurance and offer a variety of payment options to patients.  

Choose a payment/insurance method below to learn more.

I have no insurance. 

If you are uninsured or not a U.S. citizen, you may qualify for state programs, financial aid, or income-based discounts and your cost may be as low as $0. Our staff can help you enroll in these programs, if eligible.

I have a Family Pact (FPACT) card. 

If you already have a teal Family PACT card, please bring it with you when you come in.

FPACT is a California state funded program that provides no-cost family planning services such as birth control, pregnancy testing, STI testing and treatment, and more.

I have Medi-Cal. 

If you have Medi-Cal insurance, you can use it at any of our health centers. You must bring your insurance card or provide your social security number in order for us to bill your Medi-Cal insurance.

We may also be able to help with applying for presumptive eligibility Medi-Cal to help with pregnancy-related services offered at our health centers.

We accept all Medi-Cal insurance, but some of the most common Medi-Cal plans are:

  • LA Care
  • Anthem Blue Cross 
  • Allied Physicians
  • Blue Shield Promise
  • Molina 
  • Healthcare LA IPA
  • Preferred IPA
I have private insurance. 

If you have insurance through Covered California, your employer, or other private insurance, please bring your card with you to your appointment. We will verify your insurance benefits to find out what services are covered. Your insurance may ask you to contribute towards the cost (also known as a copay).

We are contracted with the following insurances:

  • Anthem Blue Cross (PPO)
  • Aetna (PPO)
  • Blue Shield of CA (PPO)
  • Cigna (PPO)
  • Care 1st
  • Health Net 
  • Oscar (PPO)
  • Kaiser Medi-Cal (Medi-cal Managed)
  • Kaiser (HMO) (Contraceptive and Abortion Services only)

Most PPOs are accepted, regardless of contract. Contact our office to determine benefit verification. To learn more about what your insurance covers give us a call at (626) 798-0706 and we'll do our best to answer your questions.

If you are covered under another person’s health plan in California, like your parents or your spouse, your insurance company may send payment information about your visit to your home address. However, you may request that your health insurance plan ensure your health information stays private and secure.

I want to pay cash (self-pay). 

We accept payment for services via cash, debit card, credit card, or flexible spending accounts (FSA). Please note that we currently do not accept Apple Pay.

If you are paying out of pocket, we can help assess your family size and income to determine if you qualify for a discounted fee or any available state-funded programs.

Notice of Good Faith Estimate 

  •  You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees. 
  • If you schedule your service at least three days in advance, make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your healthcare provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service. 
  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. 
  • Make sure to save a copy or picture of your Good Faith Estimate.

Open Payments Database Notice

The Open Payments database is a federal tool used to search payments made by drug and device companies to physicians and teaching hospitals. For informational purposes only, a link to the federal Centers for Medicare and Medicaid Services (CMS) Open Payments web page is provided here:  https://openpaymentsdata.cms.gov. The federal Physician Payments Sunshine Act requires that detailed information about payment and other payments of value worth over ten dollars ($10) from manufacturers of drugs, medical devices, and biologics to physicians and teaching hospitals be made available to the public.
